JOB DESCRIPTION

SCOPE OF WORK:
The Case Manager will provide direct services to the children and families who are
identified in need of early intervention with regular home visits as directed by the
lndividual plan. The CM will also provide identified families with ides and
recommendations on how they can work every day with their child. They will assist with
computer entries as directed by the team and present ongoing communication as
needed between all service providers and the family. The CM will need to maintain an
ongoing log of where the child and family are living and what they may need assistance
with. The HELP checklist will be used on an every visit basis and will need to be
maintained as to what skills the child is able to do and what skills they still need to gain
until age 3 when this document will be passed on to the school district or LEA that will
take over responsibility. Home visit reports are written on a quarterly basis and entered
in the child's file.
Child find activities will also be completed several times a year and the Case Manager
will also be required to assist with these child find activities, if requested.
DUTIES AND RESPONSIBILITIES:
1. To provide direct services to the children and families identified in need of early intervention with regular home visits as directed by the individual plan.
2. To complete computer entries as required.
3. To provide families of children with special needs, ideas and recommendations on how they can work every day with their child to assist them in reaching their potential.
4. Present ongoing communication as needed between all service providers and family and child.
5. Maintain an ongoing log of contacts with family and child.
6. Maintain the HELP checklist in order to know where child is and to pass it on the LEA when child becomes the age to be entered into Kindergarten.
7. To write home visits reports as directed and enter them in the child’s file.
8. To assist with child find activities.
QUALIFICATIONS:
1. Must have a 4 year degree in Special Education or Elementary Education and be
able to obtain a certificate indicating these areas.
